In addition\, the application of stereotypes like laziness\, greediness\, and lack of self-discipline to fat bodies automatically assigns a sense of culpability in the form of a morality of appearance. This societal view further marks fat individuals as ‘failed’ members who are then more likely to internalize collective fatphobia into a marginalized self-identity. Therapists working with fat patients must\, therefore\, be prepared to confront their patients’ profound feelings of shame. How can we navigate this complexity? \nIn a ‘two-person psychology\,’ a framework that emphasizes the importance of the analyst’s subjectivity in the co-construction of meaning\, we need to take into account how the therapist feels about fat. Should we admit to patients that we share the same feelings about fat as everybody else\, or do we pretend that we don’t in order to be “nice”? Regardless of whether we have ever been fat\, almost all of us have experienced what it’s like to be afraid of it. How much of it\, if any\, should we share?  \nThis presentation delves into some of the countertransferential issues that surface when therapists work with fat. In the first part\, Dr. Offman will feature a paper entitled “The Intersectionality of Misogyny: On Being Fat\, Female\, and Trans. This case study explores countertransference working with a fat trans patient\, both before and after my transformation from a very fat therapist to a ‘regular’ one. \nFor the second part\, Dr. Offman will present what happened when guest-supervising a case involving a slim psychoanalytic candidate and her 400-pound patient. She is a lecturer and supervisor in the Department of Psychiatry at the University of Toronto. She is also a supervising analyst\, faculty and Board member for the Toronto Institute for Contemporary Psychoanalysis (TICP). She is the former co-chair of the Candidates Committee for the International Association of Relational Psychotherapy and Psychoanalysis (IARPP) and a current member of the IARRP Board of Directors\, where she co-chairs the International Chapters Committee. Her writing interests include themes of otherness\, queerness\, and fatness. Her papers are used to teach about working psychoanalytically with patients who identify as non-binary. Her article “Fatphobia is Real” won the Council for Advancement and Support of Education in Medicine (CASE) Gold Writing Award. She recently published her first poem\, “Why? DESCRIPTION:Sponsored by the\nWILA Alumni Association\n			\n			\n				\n				\n				\n				\n			\n				\n				\n				\n				\n				\n				\n				\n				\n				\n				\n				PROGRAM DESCRIPTION\nSaturday\, February 3\, 2024 | 10:00 am – 1:30 pm (PST) / 1:00 pm – 4:30 pm (EST) \nThe following description was provided by Martha Stark\, MD: \nI will rather boldly attempt to formulate an “overarching conceptual framework” for the working through process in psychodynamic psychotherapy and in any depth psychology for which the ultimate goal is deep and sustained healing. I will therefore be presenting a theoretical framework that outlines a series of steps whereby psychological rigidity (rigid and unevolved defense) will incrementally evolve into psychological flexibility (more flexible and more evolved adaptation). \nThe process of getting from rigid defense to more flexible adaptation is never a straight-line progression. Rather\, evolving from psychological rigidity to psychological flexibility will involve the therapist’s strategic provision of not just support but also an artfully conceived combination of challenge and support – namely\, optimal stress. \nAgainst the backdrop of empathic resonance and a collaborative alliance\, the therapist will therefore repeatedly offer the patient optimally stressful interventions mindfully formulated to generate destabilizing internal dissonance and homeostatic imbalance. In essence\, by continuously juxtaposing “healthy but anxiety-provoking challenge” of the patient’s defenses with “less healthy but anxiety-assuaging support” of them\, the therapist will be creating growth-incentivizing “mismatch experiences” for the patient – the ongoing working through of which will prompt the patient to re-equilibrate at ever-higher and ever-more evolved levels of resilience and adaptive capacity. \nIndeed\, I will hope to demonstrate that psychodynamic psychotherapy affords the patient both impetus and opportunity\, albeit belatedly\, to master traumatic experiences that had once been overwhelming – and\, therefore\, defended against – but that can now\, by virtue of the synergy between the therapist’s loving support and the patient’s underlying resilience and innate capacity to adapt to stress\, be revisited\, re-enacted\, grieved\, reprocessed\, and recontextualized\, such that thepatient will eventually be able to “reposition” herself in relation to them. She will provide considerations for a decolonizing approach to engaging with race in psychoanalytic psychotherapy. Her research and scholarship focus on immigration\, trauma\, race\, and culturally-informed psychoanalytic psychotherapy. Her publications include over 90 peer-reviewed articles and chapters in books. Recent research has explored the experience of and responses to sexual violence among racial minority immigrant communities\, and the experience of racism and related stress faced by Asian Americans. Dr. Tummala-Narra is also engaged in theoretical and clinical scholarship examining sociocultural issues in psychotherapy from a psychodynamic perspective.  \nDr. Tummala-Narra serves on the Board of Directors of the American Psychological Association (APA) Division 45 (Society for Psychological Study of Culture\, Ethnicity\, and Race) as Diversity-Immigration Member-at-Large. She has served as a Member-at-Large on the Board of Directors in APA Division 39 (Society for Psychoanalysis and Psychoanalytic Psychology)\, and as a member of the APA Committee on Ethnic Minority Affairs\, the APA Presidential Task Force on Immigration\, and the APA Task Force on Revising the Multicultural Guidelines. Atlas will examine emotional material that we unknowingly carry with us: the memories\, feelings\, and traumas that we inherit from previous generations and the link between our parents and grandparents’ history\, and our own emotional struggles. How do we inherit\, hold\, and process things that we don’t remember or didn’t experience ourselves? What is the weight of that which is present but not fully known? Often patients come to treatment depressed\, stuck trying to resolve the past and failing to meet the needs of the present. The future is absent\, or a bleak projection of what was or is. With what Dr. Weisel-Barth calls “futurevision\,” the analyst and patient are able to co-create an image of the patient’s future\, specifically one that contains the patient’s growth into a more developed self. It involves a grounded framework for addressing the complex psychodynamics of couples in highly practical ways. The model is based on three broad themes: 1) the actualization of self-experience in an intimate relationship; 2) the capacity for mutual recognition and 3) the relationship having a mind of its own. Based on his 2014 Goethe Award winning book\, A Relational Psychoanalytic Approach to Couples Psychotherapy\, Dr. Ringstrom will offer a clinically useful road map of six nonlinear steps which wed his theory and practice. \nHis model begins with the therapist’s close attunement to the “hopes and dread’s” that each partner brings to the relationship\, pointing out the couple’s “vicious circle” interactions when each partner’s developmental (transference) longings trigger painful or unmanageable (repetitive transference) states in the other. Our patients are unique individuals\, with specific\, personality-related strengths and weaknesses\, and although we may focus on a particular “disorder” that any client describes to us\, we understand that problem in the context of the person’s individuality. Dr. McWilliams discusses the history of the concepts of personality structure and disorder and their relevance to practice\, reviewing ten different psychological lenses through which personality has been viewed: temperament\, attachment\, observed clinical pattern\, defensive structure\, affective organization\, implicit cognition\, drive tendencies\, self-definition versus self-in-relation orientation\, core relational theme\, organizing developmental issue.
