Her research and scholarship focus on immigration\, trauma\, race\, and culturally-informed psychoanalytic psychotherapy. Her publications include over 90 peer-reviewed articles and chapters in books. Recent research has explored the experience of and responses to sexual violence among racial minority immigrant communities\, and the experience of racism and related stress faced by Asian Americans. Dr. Tummala-Narra is also engaged in theoretical and clinical scholarship examining sociocultural issues in psychotherapy from a psychodynamic perspective.  \nDr. Tummala-Narra serves on the Board of Directors of the American Psychological Association (APA) Division 45 (Society for Psychological Study of Culture\, Ethnicity\, and Race) as Diversity-Immigration Member-at-Large. I will therefore be presenting a theoretical framework that outlines a series of steps whereby psychological rigidity (rigid and unevolved defense) will incrementally evolve into psychological flexibility (more flexible and more evolved adaptation). \nThe process of getting from rigid defense to more flexible adaptation is never a straight-line progression. Rather\, evolving from psychological rigidity to psychological flexibility will involve the therapist’s strategic provision of not just support but also an artfully conceived combination of challenge and support – namely\, optimal stress. \nAgainst the backdrop of empathic resonance and a collaborative alliance\, the therapist will therefore repeatedly offer the patient optimally stressful interventions mindfully formulated to generate destabilizing internal dissonance and homeostatic imbalance. In essence\, by continuously juxtaposing “healthy but anxiety-provoking challenge” of the patient’s defenses with “less healthy but anxiety-assuaging support” of them\, the therapist will be creating growth-incentivizing “mismatch experiences” for the patient – the ongoing working through of which will prompt the patient to re-equilibrate at ever-higher and ever-more evolved levels of resilience and adaptive capacity. \nIndeed\, I will hope to demonstrate that psychodynamic psychotherapy affords the patient both impetus and opportunity\, albeit belatedly\, to master traumatic experiences that had once been overwhelming – and\, therefore\, defended against – but that can now\, by virtue of the synergy between the therapist’s loving support and the patient’s underlying resilience and innate capacity to adapt to stress\, be revisited\, re-enacted\, grieved\, reprocessed\, and recontextualized\, such that thepatient will eventually be able to “reposition” herself in relation to them. In addition\, the application of stereotypes like laziness\, greediness\, and lack of self-discipline to fat bodies automatically assigns a sense of culpability in the form of a morality of appearance. This societal view further marks fat individuals as ‘failed’ members who are then more likely to internalize collective fatphobia into a marginalized self-identity. Therapists working with fat patients must\, therefore\, be prepared to confront their patients’ profound feelings of shame. How can we navigate this complexity? \nIn a ‘two-person psychology\,’ a framework that emphasizes the importance of the analyst’s subjectivity in the co-construction of meaning\, we need to take into account how the therapist feels about fat. Should we admit to patients that we share the same feelings about fat as everybody else\, or do we pretend that we don’t in order to be “nice”? Regardless of whether we have ever been fat\, almost all of us have experienced what it’s like to be afraid of it. How much of it\, if any\, should we share?  \nThis presentation delves into some of the countertransferential issues that surface when therapists work with fat. In the first part\, Dr. Offman will feature a paper entitled “The Intersectionality of Misogyny: On Being Fat\, Female\, and Trans. This case study explores countertransference working with a fat trans patient\, both before and after my transformation from a very fat therapist to a ‘regular’ one. \nFor the second part\, Dr. Offman will present what happened when guest-supervising a case involving a slim psychoanalytic candidate and her 400-pound patient. For addressing the intimacies of the relational\, intersubjective field? For attunement to deep affect and the complexities of our experiential worlds? What does it mean to work with “vulnerable populations\,” with trauma\, in a time when many across the left and right claim the mantle of “victimization”? When “protecting the vulnerable” can both be a rallying cry for social justice and a logic legitimizing draconian immigration and criminal justice policies to target racialized “military age males” and “men in women’s bathrooms” to protect (white) “women and children”? Drawing on a decolonial perspective grounded in the work of Frantz Fanon\, this workshop will work-through these tensions by articulating an integrative approach to case formulation and treatment that mentalizes the intrapsychic\, interpersonal\, and sociopolitical simultaneously. Beyond case formulation and treatment\, this workshop will also provide clinicians with tools for mentalizing politically on the role of “protecting the vulnerable” outside the consulting room and make sense of how the language of trauma and victimization serve reactionary\, indeed psychopathological\, ends.
